What's the Project?

The client is a large US-based company from the healthcare market. We are starting a multi-phase project focusing on the replacement of the clinical and data platform, replacing the current system. The work will be concentrated on delivering a flexible and configurable workflow solution. You will work in a mid-size team (20-40 people), predominantly based in Central Europe, with a client in the United States. This will be an exciting, greenfield project in a highly demanding but also rewarding environment of modern healthcare, based on cloud architectures, in data-driven and AI-enhanced environment

Role Overview

We are seeking an experienced Senior to Lead-level Backend Engineer to architect and develop a sophisticated Utilization Management and Prior Authorization application using the latest .NET frameworks. This role requires deep expertise in building scalable, event-driven architectures with complex workflow orchestration capabilities to support healthcare authorization workflows and clinical decision support.





You’re a perfect match if you have:
  • Bachelor's degree in Computer Science, Software Engineering, or equivalent practical experience
  • 5+ years of professional experience with C# and .NET framework
  • 3+ years with modern .NET (Core, .NET 5+, ideally .NET 8)
  • Advanced proficiency with ASP.NET Core, Web API, and RESTful service design
  • MS SQL Server experience including T-SQL, performance tuning, and database design
  • Workflow Engines: Hands-on experience implementing state-driven workflow systems (e.g., Windows Workflow Foundation, Elsa Workflows, Workflow Core, or custom implementations)
  • Rules Configuration: Experience with rules engines and dynamic workflow configuration (e.g., Rules Engine, Drools.NET, or custom rule-based systems)
  • Message Bus Architecture: Production experience with message brokers and event-driven patterns (Azure Service Bus, RabbitMQ, MassTransit, NServiceBus, or Kafka)
  • Microservices Architecture: Deep understanding of service-oriented architecture, API gateways, service discovery, and distributed systems patterns
  • Experience with Domain-Driven Design (DDD) and CQRS patterns
  • Knowledge of containerization (Docker) and orchestration (Kubernetes)
  • Familiarity with Azure cloud services (App Services, Functions, Key Vault, Application Insights)
  • Experience with Entity Framework Core or Dapper
  • Understanding of authentication/authorization patterns (OAuth, JWT, Azure AD)
  • Background in event sourcing architectures
  • Experience with SignalR for real-time communications
  • Knowledge of API versioning and backward compatibility strategies

    Technical Leadership (only for Lead candidates): 
  • Proven ability to mentor and guide development teams
  • Experience conducting code reviews and establishing coding standards
  • Track record of making architectural decisions for enterprise systems
  • Ability to evaluate and recommend new technologies and approaches
  • Strong documentation and communication skills

    Nice to Have:
  • Previous track record with US-based client
  • Understanding of healthcare domain
  • Microsoft certifications (Azure Developer Associate, Azure Solutions Architect)
  • Experience with distributed tracing and observability (Application Insights, Seq, ELK)
  • Knowledge of performance profiling and optimization tools
  • Contributions to open-source projects
  • Speaking experience at technical conferences or meetups
  • Familiarity with AI coding assistants such as Copilot or Cursor

Your day-to-day activities:
  • Design and develop high-performance backend services using .NET 8+ and ASP.NET Core
  • Architect and implement state-driven workflow engines with rules-based configuration systems
  • Build and maintain event-driven architectures using message bus patterns (Azure Service Bus, RabbitMQ, or similar)
  • Develop and optimize complex database solutions using MS SQL Server.
  • Design microservices architectures with proper service boundaries and inter-service communication patterns
  • Participate in implementing CI/CD pipelines and maintain code quality standards in Azure DevOps (ADO)
  • Collaborate with cross-functional teams to deliver scalable, maintainable solutions
